Rehabilitation & Skilled Care Services
Fast Track Rehabilitation Program
Our innovative Fast Track program is designed to help knee and hip replacement patients get “back on their feet” and back home as quickly and efficiently as possible — often within a week! Our interdisciplinary approach coordinates Physical and Occupational Therapy, Nursing, and Dietary specialists monitoring all aspects of recovery, with patients participating fully in the process, from initial evaluation through resumed independence. In addition to providing a speedier recovery, the Fast Track program also helps reduce the need for homecare once patients have returned home.
Following joint replacements, fractures, back injuries, and amputations, our interdisciplinary team of nurses, physical and occupational therapists, and discharge planners work together to assess your needs and develop individualized plans of care. The mission of the team is always to expedite recovery, maximize functional independence, and facilitate a safe discharge back home.
Everyone reacts to pain differently. At St. André Health Care, we provide compassionate, personalized pain management to help you achieve maximum comfort and mobility.
Therapists in our restorative care program work with residents on ambulation skills, strengthening exercises, and flexibility to help them maximize their level of independence and quality of life.
For those who have lost cognitive functioning due to illness or injury, we offer therapy and training to restore and improve attention, concentration, memory, visual-motor coordination, and more.
Centered on each client’s individual needs, our Occupational Therapists help with activities of daily living, such as feeding and dressing. OTs can provide equipment to assist with activities, offer expertise in modifying the environment to maximize independence, or offer education in energy conservation, task simplification, and adaptive equipment, such as shower benches.
Our Physical Therapists work with patients affected by a range of physical issues — arthritis, osteoporosis, cancer, hip and joint replacement, balance disorders, incontinence, etc. — in developing a specialized program to help restore mobility, reduce pain, and increase fitness levels.
Pulsed Shortwave Diathermy(SWD)

Therapeutic Exercise can help a resident regain strength, endurance, balance, and flexibility. One way for our residents to partake in effective Therapeutic Exercise is our Omnicycle. The Omnicycle is basically an ordinary excersice cycle except for it's "smart assist" technology that senses how much help the user needs to exercise. It can be operated by both the arms and legs and is ideal for residents with weakness, loss of movement, poor balance, reduced endurance, and loss of flexibility.
Speech and language therapy is available for those who have impaired communication skills due to a stroke, brain injury, or developmental problems.
Our interdisciplinary team of nursing and rehab professionals treat people with COPD, pneumonia, post thoracic surgery, and other respiratory issues. The program focuses on decreasing pain and discomfort, maximizing cardio-pulmonary function, optimizing independence and mobility, preventing recurrence of disease, and assisting with safe return home.
Whether you are recovering from a hospital stay or require ongoing intravenous therapy, our skilled nursing staff are available with expertise and a caring touch.
Comprehensive Wound Care
Diabetes, infections, post-surgical wounds, ulcers, and more can all be painful and difficult to care for. Our skilled nursing staff is available for treatment and education to promote optimal recovery.
We offer complete support and education for patients learning to live with diabetes, from understanding medications to realizing the importance of good nutrition and physical exercise.
Healthy eating is essential to a healthy life. At St. André Health Care, residents enjoy delicious, healthy meals designed by our nutrition experts and prepared by talented chefs. We are glad to accommodate special dietary requirements as well as help those in need of nutrition counseling and education.
Fall Prevention Program
Falls can be dangerous, but many times they can also be avoided. Our trained staff can help in a number of ways, from instituting a regular exercise program and checking vision and medications to offering suggestions for making surroundings safer.
Home Safety Evaluations
It might be the last place we expect to feel unsafe, but for aging individuals, home can present a number of hazards. Our caregivers will visit your home to evaluate a variety of elements — from stairs, handrails, thresholds, and rugs to lighting, heating, plumbing, and more — to determine if your loved one is safe at home without a caregiver present.
To get out of the hospital and back on your feet as smoothly as possible, we work with you and your primary physician to facilitate your discharge from the hospital and help you transition smoothly into a new regimen of care. Our interdisciplinary team of medical, skilled nursing, and rehab professionals work together with your physician to assess your needs and implement a plan of care that promotes optimal recovery.
Our interdisciplinary team is also skilled in helping patients manage ongoing health and wellness issues, including:
- Specialized Medication Management
- Chronic Disease Management
- Infection Control Management
